Professional Experience:
Dr. Takata attended the University of Hawaii and received his medical degree from the John A. Burns School of Medicine in 1991.
In 1992, Dr. Takata completed a one-year transitional medical internship with the John A. Burns School of Medicine in Honolulu, HI. After completing a three-year residency program in Physical Medicine and Rehabilitation at the Mayo Clinic in Rochester, MN, he completed a one-year fellowship in Electromyography at Mayo Clinic Scottsdale. In 1996, he joined the staff at Mayo Clinic Scottsdale in the departments of Physical Medicine and Rehabilitation and Neurology (EMG Lab) and served as assistant professor for the Mayo Graduate School of Medicine until July 2003.
In 2003, Dr. Takata returned home to Hawaii to join Physiatry and Rehabilitation Services.
